| 内容記述 | 本研究は、後輩助産師へ助産診断と技術の伝承をするために、開業助産師の熟練したわざや技能に含まれる助産行為を言語化することで助産診断・技術のポイントを明らかにすることを目的とした。開業助産師1名にインタビューを行い、印象が深く記憶にとどまっている事例について語ることを求めた。分析はOrlando,I.J.(1972)の「看護過程記録」を参考に「助産師が産婦・家族に関して知覚したこと」「助産師が知覚したことについて考えたこと、感じたこと」「助産師が産婦・家族に対して言ったこと、行ったこと」以上3つの視点で行った。事例は、長時間立位で努責し、膣壁裂傷と血腫形成がおこった20歳代初産婦であり、助産診断・技術のポイントを分析した結果、以下の3つの知見が得られた。1. 産婦が長時間の立位をとる場合には、立位の持続時間、努責の時間と強さ、陣痛の持続時間と強さ、自分の身体感覚に集中する産婦の精神状態に注意して観察する必要がある。2. 分娩第二期に突然多量の出血が生じた場合は、血液の排出状況、色、量を観察し、出血の出現時期、分娩経過をふまえて出血原因を予測する。3. 褥婦が分娩後に局所の疼痛を訴えた場合は、訴えの内容、訴えるときの表情、声色、声のトーン、繰り返し訴える様子に注意を払い、訴えに関係した局所の観察、分娩経過、分娩時処置の有無を考慮し原因を推測、緊急性の有無を判断する。 | |||||||||||||||||||||||

| 内容記述 | This research was aimed at identifying the point of midwifery diagnosis and skills by verbalizing midwifery practice. We interviewed an experienced community midwife about her most impressive case. We made an analysis of this case based on Orlando's(1972)"Nursing process record".The case described above was about a primipara in her twenties who had vaginal laceration. She had hematoma by standing for a long time. The analysis showed the following points:1. If a woman in childbirth is in a standing position, we should observe duration of her standing position, duration and strength labor in the bearing down.2. When heavy bleeding happen to her in the second term of giving birth, we should observe the condition of bleeding color and volume of the blood. We must judge cause of bleeding in consideration of time when the hemorrhage happened and the birth condition.3. If the woman after birth suffer pain, we should judge whether we should give urgent medical treatment or not, based on assessment cause of the pain, condition of the suture after birth and expression of her face.In order to pass down the skills from experienced midwife to young, we should collect more instructive cases. | |||||||||||||||||||||||
