 note = {近年、背上げによる「ずれ」が褥瘡の原因として注目されている。そこで、現場ではギャッチベッドによる背上げ行為がどのように行われているのか、その実態を明らかにした。病院や特別養護老人ホーム、老人保健施設で、看護や介護をしている536名を対象に、独自の質問紙調査を実施した。その結果、経口食事介助時の背上げ角度は60度が最も多く、食後の背上げも含めた合計時間の平均は約80分であった。また、経鼻経管栄養の背上げ角度では30度が最も多く、経鼻経管栄養後の背上げも含めた合計時間の平均は約130分で、食事に要する背上げが長時間続行していた。さらに、梶痛予防に適切な背上げ角度を知っている者は約5割、体位を整える際、臥床位置を頭側に移動している者が約5割いた。「ずれ」を防止するためにベッドの膝上げ機構を使用し、膝上げをしている者が約4割、背上げ角度の確認では、経験や感覚を拠に実施している者が約7割、背抜きを実施している者は約4割であった。本調査から、背上げ行為の一連の過程が必ずしも適切に実施されていない実態が明らかになった。, Recent advances in pressure ulcer study show that shearing force is also an important factor on the development of pressure ulcers. Slides of the body are often given when the backrest of the bed is elevated during feeding of a bedridden patient; therefore, we surveyed the degree of backrest elevation at hospitals and care facilities. Five hundred and thirty-two caregivers answered our questionnaires (collection rate: 65%). Most freqent backrest elevation angles were 60 degrees for oral food intake, and 30 degrees for tube feeding, while duration of elevated backrest position (including after feeding) was 80 min and 130 min, respectively. Caregivers who knew the appropriate backrest elevation angle for preventing pressure ulcers was 50%. Forty-eight percent of caregivers considered the appropriate lying position before backrest elevation. Forty-three percent of caregivers used knee-up function of the bed to prevent the body from sliding down during backrest elevation. Sixty-nine percent of caregivers estimated the backrest angle from their experience or feeling. Thirty-eight percent of caregivers did a body stress-decreasing procedure soon after backrest elevation. In summary, some caregivers did not have enough knowledge about the backrest elevation process and did not always use the appropriate procedure.},
