| 内容記述 | 本研究の目的は、終末期心不全状態にある患者へのEnd of Life Discussion（EOLD）の実践とその困難感を測定する尺度の妥当性と信頼性を検討することである。分析対象は全国の医療施設から無作為に抽出された141 施設の看護師188 人であった。調査期間は2021年7月の1か月間。独自に開発したEOLD 項目について、その実践頻度と困難感を調査した。分析は、まずEOLD の実践頻度のデータを用い探索的因子分析を行い、次に抽出された因子構造を用いて確認的因子分析を行った。外的側面からみた妥当性の検討は、スピリチュアルケア実践との関連で検討した。探索的因子分析の結果、「医療者チーム」「患者・家族」の2因子が抽出された。確認的因子分析では、「EOLD」の実践と困難感ともにモデルのデータへの適合性は、統計学的許容水準を満たした。ω信頼性係数も良好な値であった。今後は本尺度の一般化妥当性の検討、ならびに「EOLD」の関連要因の検討が必要である。 | |||||||||||||||||||||

| 内容記述 | The objective of this study was to examine the validity and reliability of a scale for evaluating the implementation of and difficulty in end-of-life discussion （EOLD） with patients with end-stage heart failure. The analysis included 188 nurses from 141 randomly selected medical institutions nationwide. The investigation was conducted over the course of July in 2021. For our independently developed EOLD items, the frequency of implementation and difficulty were examined. We first performed exploratory factor analysis using the data on the frequency of EOLD implementation and then confirmatory factor analysis using the extracted factor structure. External validity was examined in relation to the implementation of spiritual care. The exploratory factor analysis gave two factors: “team” and “patient/family.” In the confirmatory factor analysis, the fit of the model to the data met the level of statistical tolerance for both EOLD implementation and difficulty. The omega （ ω ） reliability coefficient was also favorable. Further studies are needed to examine the generalizability of this scale and factors related to EOLD. | |||||||||||||||||||||
