本研究は、看護師が認識している高齢末期心不全患者の心理的苦痛およびそれに対する看護実践、看護実践の中で感じる困難さの背景を明らかにすることを目的とした。循環器科を診療科とする医療機関に勤務する看護師を対象に無記名自記式質問紙調査を実施し、199 人を分析対象とした。調査内容は、心理的苦痛についての認識、心理的苦痛に対する看護実践、心理的苦痛に対する看護実践の中で感じる困難さの背景とした。結果、看護師は、高齢末期心不全患者が【苦痛から逃れられない辛さ】【自立の喪失】【予後への不安】【選択権や決定権の喪失】等の心理的苦痛をもつと認識していることが明らかとなった。また、心理的苦痛に対する看護実践として、【身体的苦痛を緩和する】【患者の全人的理解を深めるために多職種で情報共有する】【今後の療養や治療に関する自己決定を支援する】等の実践が明らかとなった。心理的苦痛に対する看護実践を行う中で感じる困難さの背景として、【治療と緩和ケアのアンバランス】等が明らかになった。緩和ケアの提供にあたって課題があるが、早期から苦痛緩和に焦点を当てたケアを提供できるよう、多職種チームで対応を検討することや、患者が今後の療養について考えることができるよう意思決定支援が重要であることが示唆された。 |

This study aimed to understand the psychological distress in elderly patients with end-stage heart failure and clarify the roles of nurses involved in their care and the difficulties experienced by nurses during the care. An anonymous self-administered questionnaire was administered to nurses working in cardiovascular departments in medical institutions. A total of 199 nurses’ responses were included in the analysis. The contents of the questionnaire included awareness of the psychological distress of their patients, roles of nurses involved in their care, and difficulties experienced by nurses during the care. The analysis revealed eight categories of nurses’ awareness of psychological distress of their patients (“distress of inescapable suffering,” “anxiety about prognosis and death,” etc.)，nine categories of the roles of nurses involved in their care (“alleviation of physical distress,” “supporting self-determination in treatment decisions,” etc.)，and seven categories of difficulties experienced by nurses during care (“balancing between curative and palliative care,” “awareness of not being good at caring for psychological distress,” etc.)．The transition from curative to palliative care is difficult for heart failure patients. They tend to express their psychological distress when physical distress has been alleviated; therefore, alleviation of physical distress is important not only for its physical need but also for the understanding and alleviation of psychological distress. |
