| 内容記述 | 本研究は、倫理的問題を認識し倫理的ジレンマに直面した際のがん看護に携わる看護師の倫理的意思決定過程を明らかにすることを目的とする。研究方法として、がん看護に携わる認定看護師に半構造化面接を実施した。分析は、質的帰納的に行った。結果として認定看護師の倫理的ジレンマに直面した際の倫理的意思決定過程は、10 カテゴリーが抽出された。認定看護師は、【倫理的ジレンマへの気づきとわだかまり】に直面し、【専門的・個人的価値基準の想起】を行いながら、【患者にとって最善な選択への自問】、【対立する価値観、倫理原則の分析】、【患者の現状把握と看護上の問題の把握】、【相手の価値観へのアプローチとリード】、【問題解決を目指したチームや他職種の巻き込み】といった問題解決行動をとり、【相手の価値観や希望を尊重する決断】に至っていた。一方、認定看護師は【困難状況による介入の挫折】も経験していた。その上で、【自己の看護の内省】を行いながら、倫理的感受性や価値基準を醸成するという循環するプロセスを辿っていた。認定看護師の特徴として【困難状況による介入の挫折】で立ち止まることなく、【自己の看護の内省】に至っていたことから、経験を意味づけし今後へ活かそうとする行動が倫理的意思決定過程内に含まれていることが明らかになった。 | |||||||||||||||||||

| 内容記述 | This study aimed to identify the ethical decision-making processes in nurses involved in cancer nursing when they recognize ethical issues and face ethical dilemmas. Semi-structured interviews were conducted with certified nurses involved in cancer nursing as study participants. Qualitative and inductive analyses were performed on the obtained data. Ten categories were identified in the ethical decision-making processes of certified nurses when faced with ethical dilemmas. The certified nurses were faced with “Awareness regarding ethical dilemmas”, and while “Recalling professional and personal values”, they took problem-solving actions, such as “Asking themselves what is the best choice for the patient”, “Analyzing onflicting values and ethical principles”, “Understanding the patient’s current situation and nursing issues”, “Approaching and leading other people’s values”, and “Involving the team and other professionals to resolve the issue”, before reaching a “Decision that respects the other people’s values and wishes”. However, the certified nurses also experienced “failure to intervene due to difficult situations”. Moreover, they followed a circulating process of fostering ethical sensitivity and value standards while engaging in “Self-reflection of their nursing”. This study revealed that nurses’ ethical decision-making processes comprised the four factors determining ethical behavior as proposed by Rest. However, the certified nurse’s characteristically did not stop at “Failure to intervene due to difficult situations” but engaged in “Self-reflection of their nursing”, indicating that their ethical decision-making processes include actions to comprehend their experiences and use them in the future. |
